Whether you prefer to walk in at one of our Health Centers or schedule an appointment closer to home, the choice is yours!

As a valued member of the NYHTC Employee Benefit Funds, you now have access to numerous local Quest Diagnostics locations, many conveniently located near your neighborhood health centers. That means you can complete your lab testing close to where you live, work, shop, and see your doctor.

More Locations, More Options: You’re no longer needed to visit a Health Center—Quest Diagnostics has numerous testing sites near you.
Online Scheduling Made Simple: Book your appointment online at a time and place that fits your schedule.
Early Morning Fasting Appointments: Need fasting labs? Now you can complete them early at a Quest Diagnostics site near your home—no extra travel needed.
Walk-In Access Still Available: Our health center is here to help take care of your lab needs when you visit your doctor.

How to Create Your MyQuest Account

Go to myquest.questdiagnostics.com/web/create-account.

Enter a unique username (you can also use your email address).

Your password must be at least 8 characters long and include at least one letter and one number.

Step 1: Enter your personal details.

Step 2: Choose which notifications you’d like to receive from Quest.

Review your information and click “Create Account.”

How to Schedule an Appointment with Quest Diagnostics

Scheduling your lab work is quick and easy! Just follow these simple steps:

Go to the Appointment Page
Visit QuestDiagnostics.com/appointment
Enter Personal Information
Provide your name, date of birth, and contact details.

Select Your Reason for Visiting
If you’re unsure, choose “All Other Tests.”

Choose Location and Time
Enter your ZIP code to find nearby Quest locations, and select a convenient date and time.

Provide Insurance Information
Use your blue and white Health Center ID card (starting with FND) to avoid being billed. Be sure to bring it to your appointment.

Review and Confirm
Double-check all details, then confirm your appointment.

💡 Important Reminder:
If you visit a Quest Diagnostics location outside of a Health Center, don’t forget to bring your Health Center ID card (FND) with you. This ensures your lab services are billed correctly and covered.

Register a Dependent on MyQuest

While you are logged into your MyQuest account, select the green circle in the upper-right corner to select “Account Settings“

Select “Manage My Circle”

Click “Add A Family Member”

Select who you are registering. For children, click that you are creating a dependent account.

Fill out all of your dependents information, accept the terms and attestation. Once completed, the child will now be under your account.
